    Essay (Recommended)

Though you aren't technically required to answer our essay questions, we highly recommend that you do. We want to learn a little about you as a person and this is our way of doing it.

We accept an essay based on our prompts or a paper you have written for class. Here are the current essay prompts:

    1. Discuss a meaningful contribution you have made through involvement in school, church or community activities.
    2. Describe an event, a person or an educational experience that has had a major impact on your life and why.
    3. Share additional information about your personal story that would be beneficial to the application review process. What do you want the readers to know about you apart from courses, grades and test scores?

We recommend that your essay be one to two pages long.

We also ask a second essay question about your experience with diversity and inclusion. This essay is also optional, but will give you a chance to reflect and speak from your heart.

    Other Materials (Optional)

Letters of Recommendation and Test Scores (Optional)

You're welcome to submit any letters of recommendation, official (school code: 2102) or (school code: 6110) test scores or any other materials you think will enhance your application.

Contact your admissions counselor for advice on what to submit.

Code of Conduct (Subject to Future Request)

The University of St. Thomas may require you to submit a Code of Conduct form. The Office of Admissions will contact you directly after an initial application review is completed if this form is needed. The form requests information about a student's disciplinary history. Most commonly, it should be completed by you and your most recently attended U.S. college/university.

If admitted, you will also be asked to complete a prior delinquency/criminal history review. Admitted students will be contacted about this review after an offer of admission is extended. All offers of admission are contingent on successful clearance of the prior disciplinary and delinquency/criminal history review, as determined solely by St. Thomas.

To fill out the form electronically, please use the "Fill & Sign" option in Adobe Acrobat.
